Complex Cases

Because of Dr. Clyburn's expertise in total joint replacement, many physicians refer hip cases which may be too complex for them to handle. These are just a few interesting examples:

Acetabular Reconstruction

In this case a previous total hip has failed leaving a large defect in the bone of the pelvis where the socket (acetabulum) should be.

A special three dimensional CAT Scan was done and a plastic model constructed using the CAT Scan data.

This allowed us to build a custom socket to fit the defect as seen in the post-operative x-ray.

Achondroplasia

The second case is that of a patient with a condition called Achondroplasia which is a form of dwarfism. It is not uncommon for patients with this condition to develop severe arthritis of the hips early in life. This patient had had both hips replaced with cemented hips. The cement of the right hip has failed and loosened as seen in the x-ray. This hip was removed and replaced with a special extra-small uncemented (porous coated) implant.

Juvenile Rheumatoid Arthritis

These x-rays are of a 16 year old girl with Juvenile Rheumatoid Arthritis. This form of arthritis is very destructive leading to small stature and advanced arthritis at an early age. This patient was in a wheel chair and desired to walk across the stage to receive her high school diploma. Both hips were replaced at one surgery using uncemented implants.

Complex hip cases
This series of x-rays are those of a patient with renal osteodystrophy. This condition is the result of kidney failure and made more severe by the steroids necessary in treatment. The bones of the legs are very bowed, osteoporotic and the bone of the heads of the femurs has died. Replacements had to be carefully sized to fit into the large diameter bone, but had to be extra-short to fit into the markedly bowed femurs.
